Maximum LOA time per year is 30 days which can be extended up to 2 weeks in case of some important personal circumstance.
If you need to go on an extended LOA fill out the Purusha Extended LOA Application and call the LOA office for approval.

LOA Notification Form: Fill out sometime prior to actual departure with estimated dates of LOA.
LOA Actual Departure Date Form: Fill out when you are sure of your actual departure date.
LOA Actual Return Date Form: Fill out when you return.
Extended LOA Form: Fill out if you need to go on an extended LOA (over 30 days).
